This article proposes the first-ever diagnostic criteria for isolated perianal Crohn’s disease (ipCD) — a condition that’s long been overlooked in clinical practice. These criteria represent a critical first step toward standardising diagnosis, improving earlier detection of IBD in fistula patients, and supporting future research to improve our understanding and management this condition.
